Enclomiphene vs clomiphene: what each name means

Enclomiphene is the trans-isomer found in clomiphene citrate, not a generic form of Clomid and not an FDA-approved drug.

Clomiphene citrate is a two-isomer mixture: enclomiphene, the trans-isomer, and zuclomiphene, the cis-isomer. Clomid is the FDA-approved version, indicated for ovulatory dysfunction in women. How enclomiphene works: the HPG axis in plain terms

Enclomiphene is a SERM, so it blocks that estrogen feedback at the hypothalamus. The brain reads the loop as under-supplied and pushes LH and FSH up, and if the testes can still answer, endogenous testosterone production rises. That's why men worried about testicular atrophy and sperm suppression look at a SERM instead of a gel.

In secondary, or hypogonadotropic, hypogonadism, the central signal is low or inappropriately normal, so raising it is coherent. In primary hypogonadism, LH and FSH may already be high because the testes aren't answering, and more signal into a receiver that isn't listening rarely helps.

1. Does enclomiphene actually raise testosterone?

The clearest benefits of enclomiphene citrate in trials are biochemical: higher total testosterone, LH, and FSH in selected men.

In a randomized six-week study, 48 men with secondary hypogonadism took daily enclomiphene or used testosterone gel, and on enclomiphene total testosterone, LH, and FSH all rose. Men on the top dose went from an average morning total testosterone of 253 ng/dL to 604 ng/dL, against 500 ng/dL on the gel: comparable levels, reached by opposite mechanisms. Read by response rate, 100% of the men on 25 mg who completed the study held a mean total testosterone above 350 ng/dL, compared with 85% on the gel, and the higher tiers tell the same story: every 25 mg completer also held above 400 ng/dL and two in three above 500, against 77% and 46% on the gel.

The gel's issue wasn't the level it reached, it was consistency. It scattered readings both below 300 and above 1,000 ng/dL, and over a full day at week six its swing averaged 590 ng/dL against 314 ng/dL on enclomiphene 25 mg (P < 0.001), swings the authors suspected came down to the realities of applying a daily gel. Comparable levels, steadier line.

Two larger double-blind trials then randomized 256 overweight men with secondary hypogonadism to enclomiphene, topical testosterone gel, or placebo for 16 weeks. Both active treatments raised total testosterone. LH and FSH climbed on enclomiphene and fell on the gel.

A 2025 review covered 10 short clomiphene or enclomiphene trials across 819 men. Its hormone findings, including higher free testosterone and dihydrotestosterone (DHT) against placebo, belong to SERMs as a class rather than to enclomiphene alone.

The enclomiphene testosterone response is real and it repeats across the trials. It isn't universal. In a small six-month study, two of seven men on enclomiphene never reached 300 ng/dL, which is what follow-up labs are for.

2. Your own signal stays switched on

Unlike exogenous testosterone, enclomiphene increased rather than suppressed LH and FSH in the published comparisons.

The effect also didn't vanish overnight: a week after the last dose, testosterone and LH were still elevated in the men who had been on 25 mg, and still above the gel group.

Enclomiphene results land on a lab panel first, so follow-up bloodwork, not guesswork, is how a response gets judged. One morning draw is enough to do the judging: at steady state on 25 mg, a single morning total testosterone tracked the full 24-hour average nearly one to one (R = 0.93).

Trials ran oral doses from 6.25 mg to 50 mg daily, all research exposures. Without an approved label there's no official enclomiphene dosage to copy, which makes strength and schedule a prescriber's call.

Enclomiphene half life sits at roughly 10 hours on FDA's reading of the trial pharmacokinetics. European regulators found the data too thin to guide dosing in older men, in renal or hepatic impairment, or in poor CYP2D6 metabolizers. Serum levels in the six-week study hadn't returned to baseline 24 hours after a dose, so it does accumulate. Vendor pages tend to say the opposite.

In the 2025 pooled analysis, SERM therapy as a class kept sperm concentration roughly 70 million/mL higher than testosterone gel did, and men on it were a tenth as likely to fall below the 15 million/mL line. What the trials don't show is that every man stayed fertile, or that enclomiphene treats male infertility.

If you want kids in the next few years, say that before you start any hormonal treatment. Fertility preservation may mean a semen analysis or a specialist referral first. "Probably fine" is not a fertility plan.

Enclomiphene side effects: what the trials reported

Most reported enclomiphene side effects in the trials were mild, and the serious signals remain open questions rather than established risks.

In the 16-week trials, about one man in five had an adverse event judged related to study drug, none severe or serious, with no significant difference between the enclomiphene, gel, and placebo groups. The most frequent events across the record were headache, hot flashes, nausea, and muscle spasms. A logged event is not a proven cause.

The open questions are the ones short, underpowered trials can't close: regulators flagged cardiac and clot-related events, changes in estradiol, hematocrit, and PSA, and eye-related events, and with no approved label, monitoring is a clinical judgment call. On the other side of the ledger, FDA's adverse-event database held zero enclomiphene reports through late 2021, with the caveat that compounded drugs are poorly captured there, and a seven-year retrospective of clomiphene, the parent mixture, found 9% of men reporting side effects, none significant.

Expect a prescriber to go through your medications, fertility goals, cardiovascular and clotting history, vision, prostate risk, and kidney and liver function. And get urgent care, not a forum thread, for symptoms that could mean a clot, cardiac event, stroke, or sudden vision problem.

Is enclomiphene FDA approved? The compounding reality

No, enclomiphene is not FDA-approved, and a compounded enclomiphene prescription is not an FDA-approved generic.

The regulatory record is short and public, and it explains why access runs through compounding rather than a pharmacy shelf. Repros Therapeutics filed enclomiphene, then called Androxal, and took a Complete Response Letter in 2015. FDA's read was that the pivotal-trial design couldn't demonstrate clinical benefit, plus concerns about entry criteria, titration, and bioanalytical validation. The European Medicines Agency landed in the same place in January 2018 with a negative opinion on insufficient safety and efficacy evidence.

Now the part most pages get wrong. FDA's 2022 evaluation recommended against adding enclomiphene citrate to the final 503A Bulks List. The certified minutes of that meeting describe the nominated substances, enclomiphene among them, as neither USP-monographed nor components of FDA-approved drugs, which is why it needed nominating at all. The advisory committee voted 4 in favor of listing and 8 against, citing limited clinical efficacy evidence.

As of May 14, 2026, FDA still lists enclomiphene citrate in 503A Category 1, meaning under evaluation. This is the 503A track: patient-specific prescriptions dispensed by state-licensed pharmacies, distinct from the Section 503B outsourcing-facility pathway. Under an interim policy, FDA says it generally does not intend to take enforcement action against qualifying 503A compounders using Category 1 substances when the guidance conditions are met. That's enforcement discretion while the evaluation runs, and it is not approval.

You'll also see it argued that enclomiphene qualifies under 503A because it is a component of approved clomiphene. That argument came from the pharmacy that nominated the substance and from one committee member on the losing side of the vote; FDA's certified minutes say the opposite.

Physique outcomes have a single data point, a 48-week trial in which testosterone rose significantly at both doses while lean body mass and strength endpoints matched placebo. So judge your response on follow-up labs and how the next few months actually go, not on a promise.

Second, the mechanism draws its own boundary. Enclomiphene amplifies a signal, so it needs testes that can still answer. Low testosterone driven by primary testicular dysfunction, obesity, sleep problems, medications, or illness runs through a different fix, and sorting out which picture is yours is exactly what bloodwork and a provider are for.

Third, the long-term record is short: the largest safety study ran 26 weeks, the longest trial 48, and bone mineral density is still an open question. The same seven-year clomiphene review that logged few side effects also found over 80% of men holding total testosterone above 450 ng/dL, with 78% reporting subjective improvement, in an uncontrolled chart review. The AUA guideline puts the diagnostic cutoff at 300 ng/dL, and the pivotal trials wanted a low early-morning total testosterone confirmed on two separate draws before anyone got a capsule. LH separates a central signaling problem from primary testicular dysfunction, which is the fork in the road for this drug.

If that describes you, you know the shape of it already: months of feeling flat, a number that finally explains something, and a fertility plan you aren't willing to trade to fix it. That's the patient these trials were aimed at.
